A Family Health Organization is a group of Physicians who work together with other allied health care professionals to provide accessible, coordinated care to their patients. This provides patients with the best medical care.
We would like to introduce Dr Ekwani Solomon who is in the process of joining our FHO group.
Dr E. Solomon is now accepting patients to her practice for a potential start date on the 1st of October 2024. Interested patients can get on her waitlist by completing, signing and returning the filled enrolment form to our Waterloo location at 99 Northfield drive east. Unit 306, Waterloo, ON N2K 3P9.
Enrolment forms are available only at the Waterloo location and should be returned there as well. Patients would need a valid OHIP healthcard to register. For patients who may be transferring from a different family doctor, please request a transfer of records at the time of submitting your enrolment form.
Registration in Our FHO Entitles You to:
Urgent Care Clinic hours on evenings and weekends.
After-hours access to a Telephone Health Advisory Service whose qualified and experienced registered nurses will give you advice or refer you to medical or emergency services if needed.
Patients who are enrolled with our physicians have the benefit of being part of our Preventive Care services. This includes pap testing, mammograms, colon cancer screening and immunizations.

Our staff ensures that searches in the electronic medical records are kept up-to-date. Patients who require testing or immunizations are identified and notified by phone and/or mail.
If you have not already done so, please sign the Patient Enrolment and Consent to Release Personal Health Information Form (available in the office). By doing so you agree to use the Phoenix Healthcare Centre FHO for all non-emergency Primary Health Care.

How can I get my test results?
Your doctor will review all test results that you have done. If your doctor would like to see you to discuss the results, they will send a message to staff to call you to arrange an appointment. Most often, you will not receive a call for normal results, but you are always free to contact the office to book an appointment to review any tests that you have done.
Can my doctor fill out this form?
If you have any forms that you need the doctor to fill out, please bring them into the office as soon as you receive them. Form completion can take up to 60 days. Please make sure that all areas that pertain to you (e.g., name, health care number, address, signed consent etc.) are filled out before handing them in. Please note that there may be a charge for completion of the form.
How do I let staff know I’ve arrived for my appointment?
Upon arrival for your appointment, please check in at the front desk with your health card. Please bring your health card to every visit. If your address or phone number has changed, please advise reception staff when checking in.
What if I need a doctor when the office is closed?
If you have a life threatening medical emergency please go to your nearest emergency room. You can also call Telehealth at 1-866-553-7205 to speak to a nurse. For continuity of care, we ask that you do not use an After Hours Clinic or a Walk-In Clinic.
Can I talk to my doctor on the phone?
Patients sometimes request to speak with their physicians by telephone. Throughout the day, your physician is busy caring for other patients, consulting with other physicians and providers and completing paperwork and does not have much available time during the day. Our team of nurses and support staff are trained to help you by taking messages for the physician and assisting as much as they can while the physician may not be available.
Why do the staff want to know my health concern?
In order to see your physician, you must make an appointment. It is necessary for our staff to ask questions regarding the nature of your visit in order to assess your needs and the urgency of your appointment as well as to book sufficient time for your health concerns. Our team members are trained to follow the policies provided by your physician.
Will I be able to get an appointment today?
We offer same-day appointments to our patients with urgent needs (ie: fever, cough, sore throat, infection, pain, etc) Mon-Fri from 5:00pm-8:00pm and Saturdays 9:00am-12:00pm. We also reserve a number of appointments during regular office hours (Mon-Fri (9:00am-5:00pm) for urgent needs, although these slots are limited. This means you will not have a need to visit a walk-in clinic and will be able to keep all of your medical records in one location with your Family Physician. Please call the office to reserve your same-day urgent appointment slot. Our receptionists manage a large number of phone calls that come into the office each day. Early morning-9-10:30 and early afternoon-1:30-2:30 are particularly busy times of the day, especially on Mondays or after holidays. If your phone call is not of an urgent nature, please try to call outside of these times.
What if I can’t make my appointment?
If you are unable to attend a scheduled appointment, please phone or email the office the day before to cancel. Other patients need your appointment time. Please be considerate.
How do I get my medication refilled?
Please call our office to book an appointment with your doctor for your refills. It may take some time before you can be seen so please be patient and don’t wait until you are completely out of medication!
Do I need an appointment?
As a policy, we do not accommodate walk-in patients, and request that you call first to make an appointment. We only service patients registered to our doctors who have a scheduled appointment.
